What is a Diabetes Self-Management Program?

A Diabetes Self-Management Program involves the educator(ME) partnering with the person with diabetes(YOU)  to take action to improve your health.

The goal is to find solutions to meet your unique needs.

Once WE find solutions to help you make positive changes you will be able to improve A1C.

By just lowering your A1C by 1%, you decrease your risk of kidney disease, amputation, stroke and blindness by 40%.
